Referees are required to submit their scores on the form below and email the game sheets to Referee Coordinator
If a card is given, please fill out game sheet provided here and email to our Technical Director record reason for card. It is important any time a card is given it is recorded IMMEDIATELY after the game in case of a disciplinary challenge.
